The New York State DMV states that applicants using Form MV-82D must provide their plate number, complete the required sections of the application, and show acceptable proof of identity, such as a New York State photo driver license or non-driver ID.

The registered owner must provide acceptable identification for the duplicate registration transaction. A current New York State photo driver license, learner permit, or non-driver identification card is commonly used as proof of identity.Additional forms of identification may be accepted depending on the transaction and the documents available.
Application for Duplicate Registration – MV-82D
Form MV-82D, Application for Duplicate Registration, is the New York DMV form used when requesting replacement registration documents. The form asks for information about the registrant, vehicle registration, address, and certification of the information provided.
The MV-82D should not be used to change the registrant’s name or other vehicle information. The DMV directs customers making those types of changes to use the appropriate Vehicle Registration/Title Application instead.
